The information below includes important legal and regulatory notices related to psychotherapy services, privacy, access to records, Good Faith Estimates, professional complaints, and emergency resources.

Information We Are Required to Share With You

Mandated Information

Various state and federal laws and professional regulations require mental health providers to make certain information available to clients.

You have the right to ask questions about your care, your clinician’s qualifications, treatment recommendations, fees, privacy practices, and any aspect of the therapeutic relationship.

You are entitled to professional, considerate, safe, and respectful care without discrimination and regardless of your payment source.

Your clinician is also required to maintain appropriate professional boundaries and will not engage in sexual relationships with current clients or other relationships prohibited by applicable professional ethics or law.

Your Rights as a Therapy Client

As a client, you have the right to participate actively in decisions about your care.

You may ask questions about your treatment, discuss concerns about the therapeutic process, request information about your clinician’s training and qualifications, or discuss whether another provider may be a better fit for your needs.

You may choose to discontinue therapy at any time. When appropriate, your clinician can discuss referrals or other resources that may better meet your needs.

Psychotherapy is a collaborative process, and you are encouraged to communicate openly about questions, concerns, or changes you would like to make in your care.

Filing a Complaint

If you have concerns about your experience with Restored Path Therapy, you are encouraged to discuss them directly with your clinician whenever you feel comfortable doing so. Concerns will be taken seriously and addressed respectfully.

You may also request a referral to another clinician or discontinue services at any time.

If you believe a licensed professional has acted unethically or violated professional standards, you have the right to contact the licensing authority in the state where you received services.

North Carolina

Complaints regarding licensed social workers may be submitted to the North Carolina Social Work Certification and Licensure Board. The Board provides an official complaint process and complaint form.

Complaints may be submitted through the Texas Behavioral Health Executive Council, which oversees the complaint process for behavioral-health professionals regulated by its member boards.

Ohio

Complaints concerning licensed counselors, social workers, and marriage and family therapists may be submitted to the Ohio Counselor, Social Worker & Marriage and Family Therapist Board. Ohio provides an online complaint process through its licensing system.

Louisiana

Complaints involving licensed social workers should be directed to the Louisiana State Board of Social Work Examiners. Because complaint procedures and forms can change, clients should use the Board’s current official website for instructions.

Requesting a Copy of Your Records

You may request access to your clinical records as permitted by applicable state and federal law.

To request records, please discuss your request with your clinician during a session or contact Restored Path Therapy directly. You may be asked to specify which records you are requesting and how the records will be used.

Protecting your confidential information is important. Once records are released to another person or organization, Restored Path Therapy may no longer be able to control how that information is stored, shared, or used.

Depending on the purpose of the request, a clinical treatment summary may sometimes be more appropriate than releasing an entire clinical record.

If records are being requested in connection with a legal matter, additional clarification or authorization may be requested before information is released so that only the information necessary for the stated purpose is disclosed.

You may be required to complete a valid Authorization or Release of Information before records can be provided to another individual or organization.

Access to, amendment of, and release of records will be handled in accordance with applicable federal and state law.

Privacy & Confidentiality

Your privacy is an important part of the therapeutic relationship.

Information shared in therapy is generally confidential and will not be disclosed without your written authorization except when disclosure is permitted or required by law.

Exceptions to confidentiality may include circumstances involving suspected abuse or neglect, certain situations involving a serious and imminent threat of harm, court orders or other legally required disclosures, and other circumstances defined by applicable state or federal law.

Because privacy and reporting requirements can vary depending on the state in which you are physically located during treatment, your clinician will review confidentiality and its limitations as part of the informed-consent process

Telehealth & Virtual Therapy

Restored Path Therapy provides virtual psychotherapy services to eligible clients located in states where the treating clinician is appropriately licensed or otherwise legally authorized to practice.

At the beginning of telehealth sessions, you may be asked to confirm your current physical location. This information is important because professional licensing requirements and emergency procedures are based on where the client is physically located at the time services are provided.

Telehealth requires appropriate technology, a reasonably private environment, and an internet connection. Although reasonable safeguards are used, no electronic communication system can be guaranteed to be completely secure or uninterrupted

Additional telehealth policies, risks, benefits, and expectations will be reviewed as part of your informed-consent documentation.

Emergencies & Crisis Situations

Restored Path Therapy is an outpatient psychotherapy practice and does not provide emergency or 24-hour crisis services.

Website contact forms, email, voicemail, client portals, and routine therapist messaging should not be used when immediate emergency intervention is needed.

If you are experiencing an immediate or life-threatening emergency, seek emergency assistance through your local emergency services or go to the nearest emergency department.

Your clinician will discuss appropriate crisis procedures and emergency resources with you as part of treatment when applicable.

No Surprises Act & Good Faith Estimate

For Uninsured and Self-Pay Clients

Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use insurance for their care may have the right to receive a Good Faith Estimate of expected charges for scheduled healthcare services.

A Good Faith Estimate provides information about the reasonably expected costs associated with your care based on the information available when the estimate is created. Federal guidance requires Good Faith Estimates for qualifying uninsured or self-pay individuals and provides a patient-provider dispute process in certain circumstances.

Your Good Faith Estimate

Before beginning services, you will receive information about the fees associated with your care.

The total cost of psychotherapy cannot always be predicted in advance because each person’s treatment needs and length of treatment are different.

Your total cost may depend on factors including:

  • The number and frequency of sessions you attend

  • The type and length of services provided

  • How long you choose or need to participate in treatment

  • Missed appointments or late cancellations subject to the practice’s cancellation policy

  • Additional clinical or administrative services you request

  • Changes in your treatment needs

  • Your insurance coverage, when applicable

A Good Faith Estimate is an estimate, not a contract, and does not require you to receive services from Restored Path Therapy.

CMS explains that a Good Faith Estimate should identify expected charges for anticipated items or services based on information known at the time the estimate is prepared.

If Your Bill Is Substantially Higher Than Your Good Faith Estimate

Federal law provides a patient-provider dispute resolution process for eligible uninsured or self-pay individuals when billed charges from a particular provider or facility are substantially higher than the amount listed in the applicable Good Faith Estimate.

Fees, Insurance & Payment

Restored Path Therapy offers private-pay services and may participate with insurance plans through third-party credentialing or billing platforms.

Insurance coverage varies significantly by plan. Acceptance of an insurance company does not guarantee that a particular service will be covered or that your individual plan considers the clinician in network.

Clients are encouraged to verify benefits directly with their insurance company, including:

  • Whether behavioral health and telehealth services are covered

  • Whether the clinician is considered in network

  • Deductible requirements

  • Copays or coinsurance

  • Authorization requirements

  • Any limitations on covered services

If you choose to submit claims independently for out-of-network reimbursement, Restored Path Therapy cannot guarantee that your insurance company will reimburse some or all of the cost.

You remain responsible for understanding your individual insurance benefits and for charges that are your responsibility under the practice’s financial policies and applicable agreements.

Current private-pay rates, cancellation policies, and other financial terms will be provided before or during the intake process.

Social Media & Public Interactions

To protect client confidentiality and professional boundaries, Restored Path Therapy clinicians generally do not engage with current or former clients through personal social-media accounts.

If you encounter your clinician in public, your clinician may not acknowledge you first. This is intended to protect your privacy, not to ignore or avoid you.

You are always free to decide whether you would like to acknowledge your clinician.

Professional Boundaries

The therapeutic relationship is a professional relationship established for the purpose of supporting your mental health and treatment goals.

Restored Path Therapy clinicians maintain professional boundaries consistent with applicable ethical standards, licensing regulations, and law.

Questions about boundaries, communication, social media, gifts, public encounters, or any other aspect of the therapeutic relationship are welcome and can be discussed openly with your clinician.

Accessibility & Nondiscrimination

Restored Path Therapy is committed to providing respectful services without unlawful discrimination.

Questions about accessibility needs, communication accommodations, or other reasonable accommodations may be discussed directly with the practice.

Changes to Policies

Practice policies, fees, legal notices, and website terms may be updated periodically to reflect changes in law, professional regulations, insurance requirements, technology, or practice operations.

When changes directly affect established clients and require notice or consent, they will be communicated in accordance with applicable requirements and practice policies.
